General Clinic Information
We are located on the 1st floor of The Kirklin Clinic. Our office hours are Monday-Friday, 8:00a.m. - 4:30p.m. Please call (205) 801-7450 with any questions or concerns.

Appointments
To schedule an appointment, please call (205) 801- 7450 or 1-800-333-6543 ext. 7450 (if outside of the Birmingham area). In the event that you need to reschedule or cancel your appointment, please call either of the numbers listed above. If possible, we appreciate your cancellation at least 24 hours in advance.

After Hours and Emergencies
If you need to reach your physician after hours, please call UAB Paging at (205) 934-3411 and ask for the Endocrinologist on call. If this is a true medical emergency, please dial 911 for immediate assistance.

What to do Prior to Your Visit

  • Verify with your Primary Care Physician that your medical records have been sent to our office. Records can be either mailed or faxed to:

  • Test your capillary blood glucose before each meal and 2 hours after each meal for 2 weeks and complete the blood glucose record enclosed.

Special Instructions
Your physician will contact you directly if he/she has any special instructions regarding your visit to the Multidisciplinary Comprehensive Diabetes Clinic.

What to Bring with You
Please bring the following with you when you come to your appointment:

  • Your insurance card.
  • Any additional information that your referring physician has not already sent to our clinic.
  • Complete the enclosed medical history forms & Medication worksheet, and bring them with you to your appointment.
  • Your Glucometer and a written record of your home blood glucose test results for the last two weeks at least. Please use the blood glucose record form enclosed.
  • Please bring your actual medications (bottles, pills, etc) including insulin.
  • Significant other (e.g. spouse, parent or caregiver)
  • The Continuous Glucose Monitor Sensor placed on you by our staff the week prior to your appointment or your own CGMS device.

What to Expect During Your Visit
Your visit to the Multidisciplinary Comprehensive Diabetes Clinic (MCDC) will consist of three main components: lab work, education and consults with specialists.

  1. Lab Work & Tests – Do NOT eat or drink for 8 hours prior to your appointment. You may have sips of water if needed. Do NOT take your morning insulin but please bring this medication with you to take after labs have been drawn.
  2. Education – Breakfast will be provided while you take part in a group educational session (Eat & Learn Breakfast). A Registered Dietitian and Certified Diabetes Educator will discuss a wide variety of topics related to Diabetes and its management. Lunch will not be provided. Please feel free to bring your own, or choose from the surrounding area restaurants.
  3. Consults with specialists – Your visit may consist of consults with an Endocrinologist, an Ophthalmologist (with a dilated exam) and an Orthopedic Specialist (for foot and ankle assessment).

What to Expect After Your Visit A summary letter, containing each of our multidisciplinary team member’s assessment and recommendations for the care of your diabetes, and related conditions and complications, will be mailed to you and your Primary Care Physician. This letter will also include a copy of your test results performed in our clinic.

We intend for this to be a consultative practice. We will leave the daily care of your diabetes and related conditions to your Primary Care Physician, including the prescribing and refills of medication. In some unique cases, our doctors may ask you to follow up in one of our subspecialty clinics to address a specific issue or problem. Otherwise, your Primary Care Physician may refer you to us for a yearly comprehensive diabetes evaluation the following year.

Payment Information
Some insurance providers may require a referral or pre-certification before seeing a specialist of this nature. Please be sure to bring a copy of this referral or pre-certification with you for insurance/billing purposes. If you are unsure of what your insurance company requires, please contact them in advance of your appointment and they will be happy to answer any of your questions. Your co-pay is expected at the time of your visit; however, our financial counselors are available to discuss any problems or special circumstances. Parking Deck Location Patients visiting The Kirklin Clinic may take advantage of convenient parking in the deck adjacent to the Clinic. Located at 2100 6th Avenue South, the parking deck is connected to the second floor of The Kirklin Clinic by a weatherproof sky bridge. Please bring your parking ticket with you to your appointment and it can be validated. Valet parking is available on the 2nd level of the deck.

Patients with limitations
Patients visiting The Kirklin Clinic may take advantage of convenient parking in the deck adjacent to the Clinic. Located at 2100 6th Avenue South, the parking deck is connected to the second floor of The Kirklin Clinic by a weatherproof sky bridge. High top van access is restricted to the first floor of the clinic parking deck. Therefore, patients arriving in high top vans and requiring wheelchair assistance should be dropped off at the front door of the clinic before the driver parks the vehicle. Valet parking is available on Level 2 of the deck; wheelchairs and additional assistance are located here as well.
